Immigration Physical Information & Checklist

Overview

To complete your immigration physical, all required vaccines must be up to date. Call 770-532-0800 or email immigration@gic30.com to schedule your first appointment.

Physical Exam Process

Two visits are required: Visit 1 includes bloodwork and testing. Visit 2 includes the physical exam.

Cost

$450 includes both visits, gonorrhea test, and bloodwork for syphilis and TB. Additional tests may cost extra.

After First Appointment

Email vaccination records and a photo ID to immigration@gic30.com before scheduling your second visit.

What to Bring

Photo ID and vaccination records.

Required Vaccines

• All Ages: MMR, Tetanus, Varicella, Polio, Flu (Oct–Mar)
• 11–18: + Meningococcal
• 12–18: + Hepatitis A
• 59 & Younger: + Hepatitis B
• 65+: + Pneumococcal
