Immigration Checklist

1. To complete your immigration physical and schedule your second appointment, you must have all required vaccines.
2. If any vaccines are missing, visit your nearest Health Department.
3. If you received vaccines outside Georgia, they must be transferred into GRITS (Georgia Immunization Registry).

Required Vaccines for All Ages

• MMR
• Tetanus
• Varicella
• COVID‑19
• Flu Shot (Required during flu season: 10/1 – 3/1)

Vaccines Required for Ages 18 & Under

• All vaccines listed above
• Hepatitis B

Vaccines Required for Ages Up to 23

• All vaccines listed above
• Meningococcal

Vaccines Required for Ages 65 & Above

• All vaccines listed above
• Pneumococcal

Physical Exam Information

• The immigration physical requires 2 visits:
  • 1st appointment is for the bloodwork.
  • 2nd appointment is the physical.
• Cost: $450 for adults and children. Includes:
  • Both visits
  • Gonorrhea urine test
  • Blood work for syphilis and TB
• Additional charges may apply if further testing is required (e.g., X‑rays).
• Payment is due at the time of service.

After Your First Appointment

Email the following to immigration@gic30.com:

1. Scanned copy or photocopy of your vaccination records
2. Photo ID of the person receiving the physical

Your second appointment cannot be scheduled until these documents are received.
