Sinan Haddad, MD has been designated as a Civil Surgeon by the U.S. Citizenship and Immigration Service (USCIS) and now performs affordable, quality immigration physicals at both locations.
If Dr. Haddad is seeing patients on the day you come in to begin your physical then you may receive your physical and initial blood work/TB skin test on this day. If Dr. Haddad is not in the office on a day you would like to begin your physical the initial blood work/TB skin test can be performed. Dr. Haddad would then be able to perform your physical on the day you come back for your TB skin test reading.
Depending on the outcome of your TB and blood test, it may be possible to complete an immigration physical within several days to a week. Our Immigration physicals are $350.00 (including RPR, TB test, and GC Chlamydia).
Additional charges will apply for TB test and blood work or any other medical testing that may be required. Payment is requested at the time of service.
What to Bring
One of the main functions of a Civil Surgeon is to verify that you are current on your CDC recommended vaccinations. If you have any medical records of previous vaccinations please bring those with you. Also please bring your completed portion of your I-693 form with you.
Guilford Immediate Care will keep your immigration physical paperwork until all tests have been completed. After this the paperwork will be released to you in a sealed envelope. Please do not open this envelope as it is required that it be presented to the USCIS while sealed.
Dr. Haddad is not responsible for determining whether an applicant is actually available for adjustment in status. That determination is made by a USCIS officer after reviewing all relevant records, including the report of the medical examination.
Immigration physicals are required for anyone who would like to gain entrance and citizenship into the United States. The following vaccines are needed to complete the USCIS Immigration Medical Examination. The patient must either have proof of the vaccination or they are required to have titers drawn and results showing immunity.
- Pneumonia (if older than 65)
- Hepatitis B
- Varicella (if no proof of history)
- Influenza- only if in season (September thru April 1st)
